How to Make the Most of Your Visit With Us
Before Your Visit
  • Patient Portal: Soon after receiving an appointment, you will receive information about our patient portal with username and password information. Log into the portal and submit the patient registration form and clinical questionnaire.
  • Health Information: Obtain all prior medical records relevant to your visit. These should include recent lab results, endoscopic procedure reports, biopsy results, prior operative reports, and reports related to radiologic work up such as ultrasound, MRI, or CT scan. Specifically, if you have had a CT scan, obtain a CD copy of the images to bring with you.
  • List of Questions: Create a list of items that need explanation or clarification so that you will make the most of the time spent with your surgeon.
  • Medications: If you have a long list of medications, create a list.
  • Allergies: If you have many allergies, create a list.
  • Plan Ahead: Carve out enough time out of your day for your visit so you would not feel distracted or rushed. A typical visit for a new patient will take 60-120 minutes and for an established patient 30-45 minutes.
  • Arrive Early: Arrive at least 15 minutes early and have enough time for parking and check in.
During Your Visit
  • Items to Carry: Have the following items on you during your first visit
    • Driver’s license or government issued ID
    • Insurance card
    • List of medications and allergies
    • Copies of medical reports
  • Photograph: Be prepared to have your photograph taken for our electronic medical records.
  • Vital Signs: Be prepared to have your temperate and vital signs taken
  • Prepare for Answers: Be ready to answer questions related to your illness such as nature of symptoms, duration, intensity, frequency, exacerbating and relieving factors, response to prior to therapies, etc.
  • Ask Questions: Remember to go over all your questions, no matter how trivial they may seem. Your visit with us is designed to be fully informative and comprehensive. Ideally you should leave with a clear picture of your treatment plan.
  • Understand the Process: We value your time and make every effort to run on time. Occasionally, due to patient-related reasons beyond our control, your visit may be subject to slight delay. We thank you for your understanding and patience in such an instance.
Cancellation Policy

In the event of the need to cancel your appointment, we ask that notify us via email or telephone, as early as possible and no later than 1 day before your visit. This provides an opportunity to offer your appointment time to other patients on the waiting list.
